　　時(shí)光流逝，轉(zhuǎn)眼間我在成長中又渡過一年�；厥走@走過的一年，很榮幸能與各位同事共同進(jìn)步，我也在大家的身上學(xué)到不少的知識。一年以來我心中最大的感受便是要做一名合格的兒科主治醫(yī)師越來越難，要做一名優(yōu)秀的兒科主治醫(yī)師則是難上加難。我認(rèn)為：一名好的兒科主治醫(yī)師不僅要為人謙和正直，對事業(yè)認(rèn)真兢兢業(yè)業(yè)。而且在思想政治上、業(yè)務(wù)能力上更要專研。我，作為一名只學(xué)過一年漢語的中年兒科主治醫(yī)師，需要學(xué)習(xí)的東西還很多很多。先將這一年的工作總結(jié)如下。

　　本人同往年一樣，在院紀(jì)院規(guī)所容許的框架內(nèi)，在本人工作職責(zé)所規(guī)定的范圍之中，不顧自身健康狀況欠佳的實(shí)情，加班加點(diǎn)、盡心盡力、嘔心瀝血地工作。不論什么時(shí)候都能遵守勞動紀(jì)律，按時(shí)上下班。能堅(jiān)持一日兩次查房。認(rèn)真帶教下級醫(yī)師和書寫電子病歷。堅(jiān)持值夜班、積極診治和搶救夜班期間來的患兒。積極參加科室內(nèi)危重患兒的搶救、產(chǎn)科及其它科室的會診和科內(nèi)的各類疑難病列討論。能積極參加院內(nèi)和科內(nèi)組織的各類業(yè)務(wù)和政治學(xué)習(xí)，并共計(jì)支付一個月的工資取得上級規(guī)定的年度專業(yè)繼續(xù)教育學(xué)分和公共科目繼續(xù)教育合格證。能虛心向科主任討教，在科主任的帶領(lǐng)下能把自己份內(nèi)的工作做的盡善盡美。能獨(dú)自勝任及從容處理科內(nèi)所有診療工作。

　　今年曾多次在下夜班期間，根據(jù)患兒家長的點(diǎn)名要求及院科兩級領(lǐng)導(dǎo)的臨時(shí)安排，夜間護(hù)送其他醫(yī)生的危重新生兒上烏市上級醫(yī)院。春節(jié)值班期間，在夜間查房時(shí)我發(fā)現(xiàn)由其他醫(yī)生管理的一名患兒的癥狀和臨床表現(xiàn)與其所作診斷不符，我當(dāng)時(shí)高度懷疑為“甲流”，于是我立即請科主任會診，當(dāng)夜取標(biāo)本經(jīng)疾控部門確診為我地區(qū)今年第一列“甲流”患者。

　　在院科兩級領(lǐng)導(dǎo)的關(guān)心、關(guān)懷和照顧下，我今生有幸第一次上烏市參加“全疆危重新生兒診療新進(jìn)展”學(xué)習(xí)班，可能是因我工作至今第一次外出參加學(xué)術(shù)會議的原因，或許是我院兒科硬件及軟件建設(shè)遠(yuǎn)遠(yuǎn)滯后與其它地州級醫(yī)院的實(shí)情對我的觸動，參加完學(xué)習(xí)班回來后，我感慨萬分，一回到我院，便按耐不住激動的心情趕緊寫了“參會感想與思考”交給了院長。

　　庫爾班節(jié)上班期間，我最先觀察并發(fā)現(xiàn)我院第一列新生兒先天膈疝病列，為了明確診斷及時(shí)邀請?jiān)�疆的王琪主任會診，確診為先天膈疝后，得到援疆王琪主任的高度好評。近日在援疆王琪主任的指導(dǎo)下，我在兒科首先使用了“新生兒腸外營養(yǎng)”技術(shù)，此項(xiàng)技術(shù)對不能耐受經(jīng)腸道喂養(yǎng)或腸道喂養(yǎng)不能達(dá)到所需總熱量X%的危重新生兒患者有很好的療效。

　　我于1984年7月畢業(yè)于醫(yī)學(xué)專科學(xué)校臨床醫(yī)學(xué)(3年制)，分配到x廠(后改名為股份有限公司)職工醫(yī)院，因單位工作安排于1991.06—1991.12x市第一人民醫(yī)院在進(jìn)修內(nèi)科半年，1995年3月—1996年2月在x醫(yī)院進(jìn)修內(nèi)科1年，又于20xx年12月—20xx年06月在xx市第二人民醫(yī)院進(jìn)修心血管內(nèi)科6個月，一直在基層醫(yī)療機(jī)構(gòu)從事普通內(nèi)科診療工作的30余年中，虛心求教，發(fā)奮學(xué)習(xí)，不斷臨床總結(jié)經(jīng)驗(yàn)教訓(xùn)，立志成才，創(chuàng)新開拓，努力爭當(dāng)一名能給患者解除病痛的好醫(yī)生而不懈工作。30年來，救治患者25萬余人，平均每年到患者家中出診訪視巡診100人次以上。

　　圖儀，并開始在心電圖監(jiān)測下對有適應(yīng)癥的病人使用去乙酰毛花苷注射液稀釋后靜脈推注治療急性房顫30例，并取得了較好的療效，也得到了很多臨床資料。同時(shí)也試著選擇部分患者用三磷酸腺苷二鈉快速推注治療陣發(fā)性室上性心動過速32例，同樣取得了很好的效果。

　　20xx年11月，我所在的公司開始改制并將職工醫(yī)院改組成股份合作制門診部。由于企業(yè)位于一個鄉(xiāng)鎮(zhèn)邊緣上，周圍是農(nóng)村，離鎮(zhèn)上最近的醫(yī)院有一公里距離，為了顧全大局，我和原有的廠礦醫(yī)院的同事一起在公司改制領(lǐng)導(dǎo)小組的支持

　　下，將原職工醫(yī)院改制為門診部，開始著手門診部的建立并維持其正常運(yùn)轉(zhuǎn)，為維護(hù)公司改制這個大局做出了積極努力。后來，在這一改制后的內(nèi)江恒康門診部，我同樣一干就是五年。這五年，我積極帶領(lǐng)門診部一班人，積極為原企業(yè)改制后的職工服務(wù)，艱難地維持門診部開展正常門診診療工作，在盡心盡力地開展醫(yī)療服務(wù)工作的同時(shí)，著手尋求門診部的出路，終究等到了建立社區(qū)衛(wèi)生服務(wù)站、依托中心衛(wèi)生院院本部而成立的分院的那一天。

　　經(jīng)過當(dāng)?shù)攸h和政府、衛(wèi)生主管局和衛(wèi)生院領(lǐng)導(dǎo)的共同努力，20xx年1月26日門診部并入中心衛(wèi)生院，我負(fù)責(zé)分院的工作，帶領(lǐng)分院的職工開始了在新的單位工作。新成立的峨柴分院主要從事社區(qū)醫(yī)療和公共衛(wèi)生服務(wù)，特別是對慢性病、多發(fā)病病員服務(wù)。20xx年6月，我們根據(jù)糖尿病診療指南，開始對社區(qū)非胰島素治療效果不佳或者有并發(fā)癥，有使用胰島素適應(yīng)癥的糖尿病病人進(jìn)行胰島素的規(guī)范治療，采取先由醫(yī)院對這一類患者進(jìn)行大約為期2周的系統(tǒng)糖尿病教育，并開始評估，進(jìn)行胰島素治療，至20xx年8月我們對116例社區(qū)2型糖尿病患者進(jìn)行了比較完整的治療，改變了當(dāng)時(shí)社區(qū)糖尿病患者的認(rèn)識和治療觀念，取得了良好的效果，也產(chǎn)生了積極影響。

　　分院所在的峨柴社區(qū)和周邊農(nóng)村有十多例在上級醫(yī)院診斷明確的擴(kuò)張型心肌病的病人，就診治療均要去市里面的大醫(yī)院，十分不便。為此，我?guī)ьI(lǐng)醫(yī)院內(nèi)科醫(yī)師系統(tǒng)學(xué)習(xí)“擴(kuò)心病”知識，于20xx年2月對這些病人進(jìn)行家訪，并接受這十幾例患者在我院就診隨訪，我們采用綜合治療和隨訪觀察的方式，對他們進(jìn)行觀察治療，取得了令他們滿意的效果，至20xx年6月我們治療了社區(qū)和周邊農(nóng)村16例擴(kuò)張型心肌病的患者，也使醫(yī)院參與治療的內(nèi)科醫(yī)師得到了很大提高。

　　20xx年，我們在胰島素治療的病例中，發(fā)現(xiàn)近20例胰島素抵抗的患者，我們在查閱資料和復(fù)習(xí)有關(guān)資料的同時(shí)，開始胰島素抵抗的防治，通過加強(qiáng)糖尿病

　　病人的生活方式管理、增加胰島素增敏劑、改變胰島素制劑和改變注射部位、方式等，至20xx年8月，我主持了23例胰島素抵抗患者的治療，80%的患者取得了良好效果。

　　20xx年6月，由于工作需要，我擔(dān)任中心衛(wèi)生院業(yè)務(wù)副院長并兼峨柴分院主任。我主持了重癥老年支氣管哮喘病人的綜合治療，通過對這一類患者進(jìn)行分析評估，以補(bǔ)液、調(diào)節(jié)電解質(zhì)平衡、糾正低氧血癥、緩解患者氣道梗阻、預(yù)防病情惡化，適時(shí)呼吸機(jī)輔助通氣、適當(dāng)選擇藥物等治療，取得了良好效果，積累了很多寶貴的經(jīng)驗(yàn)，結(jié)合患者隨訪管理的方式，促進(jìn)了支氣管哮喘防治指南的實(shí)施。例如典型病例如饒文錢[病案號10254-20xx]，男，61歲。因反復(fù)咳嗽、氣喘、心累20余哮喘病史二十余年入院。住院期間檢查合并2型糖尿病。該患者屬于我院長期管理的社區(qū)病人，門診病史表明其哮喘急性發(fā)作或加重時(shí)病情嚴(yán)重，急救時(shí)對茶堿類或B-受體激動劑類藥物均不敏感，只能使用首次0.5毫克，以后每隔3-5分鐘多次0.3毫克小劑量腎上腺素皮下注射進(jìn)行急救，緩解后綜合治療。該患者來院時(shí)在門診已經(jīng)做了急救處理，住院期間患者病情基本穩(wěn)定，經(jīng)過抗炎、平喘、抗感染的綜合治療后病情改善出院。出院后在我院門診隨訪4年余病情穩(wěn)定，情況良好。

　　20xx年5月，結(jié)合我們基層醫(yī)院的實(shí)際，我主持了中西藥結(jié)合治療胃潰瘍的療效觀察工作，通過對照比較的方法，對胃鏡檢查確診的病例結(jié)合基層醫(yī)院的實(shí)際情況，在西藥治療的基層上采用中藥組方(我們稱為“螵茯?yàn)跎硿?rdquo;)治療，取得了良好效果，通過統(tǒng)計(jì)分析，與對照組有明顯的統(tǒng)計(jì)學(xué)意義。促進(jìn)了中藥科參與醫(yī)師的治療觀念轉(zhuǎn)變，使他們也了解了臨床對比統(tǒng)計(jì)的方法。

　　20xx年2月，根據(jù)《中國高血壓防治指南(20xx) 》，主持我們有條件的診治、隨訪的H型高血壓隨訪觀察項(xiàng)目，采用馬來酸依那普利葉酸片治療、隨訪觀察大約3年時(shí)間，并就降低腦卒中發(fā)生率進(jìn)行效果分析，目前我們已經(jīng)收集

　　14例在治病例，雖然還沒有明顯成果，但積累了一定經(jīng)驗(yàn)，相信會給我院帶來良好的經(jīng)濟(jì)和社會效益。
